Continuing Care

When you have made enough progress during your stay with us that you and your treatment team have determined that you no longer need the structure of a hospital setting, you will work together to determine an appropriate aftercare plan. If you feel as though you’re not quite ready to leave the structured environment of a short-term facility, you may choose to transition to a residential facility. Other people may choose to join our Partial Hospitalization Program (PHP) or Intensive Outpatient Therapy (IOP) programs. Some may opt to discharge to home. If you discharge to home, we will work with local providers to continue your treatment as well as connect you with community resources available to allow for your continuous healing.
